• Provides clinical documentation to support the claim & reimbursement.

• Submits the Superbill & documentation to the insurer.

• Receives direct reimbursement from the insurer.

Our Responsibility:

• Provides a receipt (but not a Superbill) & a GFE.

• Does not bill insurance.

• Documents care for clinical record.

Your Responsibility:

• Pays for services out-of-pocket or uses HSA/FSA.

• Receives a receipt (but not a Superbill) & a GFE.

• Does not submit a claim to the insurer.

Our Responsibility:

• Provides a receipt (but not a Superbill).

• Does not bill insurance.

• Documents care for record.

Your Responsibility:

• Pays for services out-of-pocket or uses HSA/FSA.

• Receives a receipt (but not a Superbill).
